Injection/Disposal (H-5) Application and Injection-Storage Permit Query Experiencing Issues

The Injection/Disposal Well Pressure Test (H-5) application and Injection-Storage Permit Query is currently experiencing difficulties with retrieving the records using Search. Information Technology Services (ITS) is working to resolve the issue and will provide an update as the service has been restored.

RAILROAD COMMISSION ADOPTS NEW FORMS FOR CHAPTER 4 IMPLEMENTATION

On October 28th, 2025, the Railroad Commission (RRC) of Texas adopted new proposed forms:

  • Form EP-1, Authorized Pit Registration/Change Form (Excel format)
  • Form EP-2, Schedule B Authorized Pit Financial Security Worksheet
  • Form EP-2A, Addendum to Form EP-2
  • EP-5, Permit Application for Surface Management of Oil & Gas Waste (PDF and Excel Worksheet) (Replacing Form H-11 and Form R-9)
  • Form EP-9, Generator Waste Profile
  • Form EP-10, Waste Manifest

Use of Commission-approved forms is required and effective October 29, 2025.

The newly adopted forms will be available on the RRC Oil & Gas Forms web page https://www.rrc.texas.gov/oil-and-gas/oil-and-gas-forms/ and on the Environmental Permits and Support (EPS) Chapter 4 web page https://www.rrc.texas.gov/oil-and-gas/applications-and-permits/environmental-permit-types/all-about-chapter-4/.
